Walk-in appointments: * 1:00pm - 5:00pm, Tuesday and Thursday at 6500 Humboldt Ave. N or call (763) 314-8374 Ext: 5107 * 1:00pm - 5:00pm, Wednesday at 1500 59th Ave. N To receive services, a completed and signed consent form is needed annually from the parent or guardian. Consent forms are available online, in school health offices and at the Health Resource Center.
