Case managers help people to plan, access, evaluate and coordinate services from their service providers. Plans are customized to each person's own needs and preferences. Learn more about case management costs that are covered through waivers in the Community-Based Service Manual. MORE INFORMATION Social workers can assist older adults and their family members in learning about community resources. Social workers are available to speak about concerns over the phone or meet for a face-to-face visit. Services include:

• Assessment
• Information and referral
• Short-term case management to assure that older adults are receiving the services they need to live safely

Support and information can also be provided to friends and family members who are assisting an older adult.
