Eligibility

  • Adults (18+) with a primary diagnosis of schizophrenia, schizoaffective disorder, a major depressive disorder with psychotic features, bipolar disorder, or other psychotic disorders.
  • Resident of Hennepin County
  • Engage with the team 2 - 3 times per week

Application process

Clients may be referred to the program by an insurance provider. Referrals can come from anyone, even a self-referral, as long as a client is Hennepin County of Financial Responsibility.

Site Service Area

Most programs serve Hennepin County.  Service area may serve additional metro counties and is dependent on program.

An intensive, comprehensive outpatient treatment and rehabilitative mental health services program for adults with a primary diagnosis of schizophrenia, schizoaffective disorder, a major depressive disorder with psychotic features, bipolar disorder, or other psychotic disorders. The program provides support with services in co-occurring disorder treatment, family psychoeducation and support, housing access, medication, and education.
