* Adults (18+) with a primary diagnosis of schizophrenia, schizoaffective disorder, a major depressive disorder with psychotic features, bipolar disorder, or other psychotic disorders. * Resident of Hennepin County * Engage with the team 2 - 3 times per week

Application process

Clients may be referred to the program by an insurance provider. Referrals can come from anyone, even a self-referral, as long as a client is Hennepin County of Financial Responsibility.

Description

An intensive, comprehensive outpatient treatment and rehabilitative mental health services program for adults with a primary diagnosis of schizophrenia, schizoaffective disorder, a major depressive disorder with psychotic features, bipolar disorder, or other psychotic disorders. The program provides support with services in co-occurring disorder treatment, family psychoeducation and support, housing access, medication, and education.
