Eligibility

Adults with Mental Illness (MI), Serious Mental Illness (SMI), and serious and persistent mental illness. Available in Carlton, Lake and St. Louis counties

Hours

8:00am - 4:30pm, Monday - Friday

Application process

Referral sources are the State of Minnesota Rehabilitation Services Office, Human Development Center agency providers, other mental health providers, and self-referrals. All referrals go through an assessment of needs and eligibility.
