A rehabilitation agency is an agency that provides a combination of treatment and education services to restore maximum functioning, a sense of well-being, and a personally satisfying level of independence for people who have temporary or permanent disabilities. At a minimum, a rehabilitation agency must provide:

• Physical therapy or speech pathology services
• A rehabilitation program with social or vocational adjustment services

This provider is a Medicare Certified Outpatient Physical Therapy or Outpatient Speech Pathology or Outpatient Occupational Therapy provider and is subject to periodic inspection by the Minnesota Department of Health.
