Available to individuals experiencing homelessness or are currently unhoused and are eligible for Minnesota Health Care Programs (MHCP) with a referral from a MHCP enrolled hospital, clinic or provider office. Must be 21 years or older and enrolled in Medical Assistance or 19 years or older and enrolled in MinnesotaCare.
