Eligibility

  • The primary caregiver (18+) for a person living with dementia, or a person living with dementia by themselves (no formal diagnosis required)
And
  • Lives in a private home setting (home, apartment, or independent living) within the Twin Cities Metro area

  • Free / no cost to eligible clients
Provides a free phone-based program, for up to one year, helping people living with dementia and/or their caregivers navigate care and supportive resources such as food delivery, transportation, financial assistance, support groups and more. Helps:
  • Determine immediate needs
  • Support caregiver health and well-being
  • Share education resources and community services
  • Support behavior changes
  • Provide safety screenings and recommendations
  • Assist with medication review and navigation support
  • Navigate advance care planning
