- The primary caregiver (18+) for a person living with dementia, or a person living with dementia by themselves (no formal diagnosis required)
- Lives in a private home setting (home, apartment, or independent living) within the Twin Cities Metro area

Provides a free phone-based program, for up to one year, helping people living with dementia and/or their caregivers navigate care and supportive resources such as food delivery, transportation, financial assistance, support groups and more.
Helps:
- Determine immediate needs
- Support caregiver health and well-being
- Share education resources and community services
- Support behavior changes
- Provide safety screenings and recommendations
- Assist with medication review and navigation support
- Navigate advance care planning
