Case managers help people to plan, access, evaluate and coordinate services from their service providers. Plans are customized to each person's own needs and preferences. Learn more about case management costs that are covered through waivers in the Community-Based Service Manual. MORE INFORMATION Provides case management services:
  • Medical: A collaborative process of assessment, planning, care coordination, and evaluation to provide cost-effective, quality case management services to individuals receiving disability compensation.
  • Vocational: For individuals receiving disability compensation with the goal of a quick and successful return to the date-of-injury job or to a job that, as closely as possible, restores the injured workers earning capacity.
