Most programs serve Itasca County. Service area may be dependent on program. CCBHC services available in Itasca and Koochiching counties.

Case managers help people to plan, access, evaluate and coordinate services from their service providers. Plans are customized to each person's own needs and preferences. Learn more about case management costs that are covered through waivers in the Community-Based Service Manual. MORE INFORMATION Adult Mental Health-Targeted Case Management is available to individuals diagnosed with a serious and persistent mental illness. Case Managers assist in:
  • Obtaining and coordinating needed services
  • Provide support, advocate for client rights, and work with clients toward their recovery
  • Case Managers work closely with other community providers to ensure individual needs are met and that successful community living is maintained
