Eligibility
Patients must be traveling to a cancer-related medical appointment. Patients under the age of 18 must be accompanied by a legally responsible adult such as a parent or caregiver.
Application process
Call (800) 227-2345 to find out if transportation is available in your community. Several days notice is needed to coordinate rides, please call well in advance of your appointment date.
