Services are directed at maintaining or improving a person's ability to self-care. This includes health, social and nutritional services. Services are directed toward the achievement of specific outcomes identified after assessment in a service plan. This service may be provided either in-person or remotely. MORE INFORMATION The Day Break adult day services program provides a daily alternative for individuals who require extra support to remain in the community and provides respite for caregivers. The Adult Day Service is open Monday - Friday. Daily services include: * Activities to increase physical strength and cognitive abilities * Meals * Social and recreational activities * Daily group exercise * Caregivers respite support * Transportation to and from services for clients
