Description

A community clinic that is state-certified to offer comprehensive, person-centered and family-centered, trauma-informed, evidence-based behavioral health and substance use disorder services in an integrated, one-stop-shop model. Provides a variety of services to children and adults with complex mental health and substance use disorders so that they do not have to piece together care across multiple providers, increasing the likelihood that care is received before a person enters into a crisis. Core services provide: * Crisis services available 24/7 * Care coordination with other providers and systems * Screening, assessment, diagnosis and risk assessment * Outpatient mental health and substance use disorder services * Mental Health Targeted case management for children and adults * Outpatient primary care screening and monitoring * Community based mental health care for veterans * Peer, family, and recovery support services * Person-centered and family-centered treatment planning * Psychiatric rehabilitation services for adults and children * Outpatient withdrawal management services MORE INFORMATION The Urgent Care Behavioral Health Center, part of the Clarity Project, is a collaboration between the Human Development Center, Center for Alcohol and Drug Treatment, and Children's Dental Services. The program is designed to serve those who need mental health, behavioral health, and/or substance use services. The Center helps to stabilize the individual and their family, link them with essential resources and assistance, and ensure follow-up care.
