A team of a nurse, an occupational therapist and a handy worker addresses both the home environment and uses the strengths of the older adults themselves to improve safety and independence. * Provides minor home modifications * The goal is to decrease difficulty with Activities of Daily Living, reduce symptoms of depression, and improve motivation This project is funded by a Minnesota Department of Human Services Live Well at Home Grant.
