Medica Minnesota Health Care Programs (MHCP) Members

7:00am - 8:00pm, Monday - Sunday

Application process

  • Call Provide-A-Ride at least five working days before the appointment: (888) 347-3630
  • Call Medica Member Services at the number on the back of your Medica identification (ID) card at least five days before the appointment.
