Transportation program that provides rides for people in and around their community. Learn more about transportation costs that are covered through waivers in the Community-Based Service Manual. MORE INFORMATION A Community Health Representative (CHR) can provide transportation to medical, dental, Women, Infants and Children (WIC) and optical appointments. Appointments should be confirmed with the CHR to assure that clients arrive on time to their appointment. The CHR will wait for clients and transport them back home. No one under the age of 18 will be transported without a parent. CHRs also deliver medications to homebound individuals with no other means of getting their medications.
