Community Health Workers reduce cultural and linguistic barriers to health care for refugees and immigrants. They serve as liaisons between underserved populations and the health care and social systems in Rochester, Kasson/Byron, and Austin/Albert Lea communities.
Community Health Workers:
- Assist individuals and families access to resources in the community including healthcare needs and food
- Provide health education about wellness and staying healthy and managing your chronic condition
- Help enroll individuals onto a health insurance plan
- Connect individuals to culturally appropriate health care services and resources
- Help individuals and families understand their health insurance policy and what it covers
- Provide individuals with informal counseling, support, and follow-up
- Help individuals understand and manage their health condition by developing strategies and habits to improve their health and well-being
- Provide referrals to community resources
- Coordinate care between different providers
