Eligibility
  • Individuals who have experienced serious and persistent mental health problems
  • Individuals who would benefit from Community Support Plan (CSP) services
  • Residents of Lincoln, Lyon, Murray, Redwood or Yellow Medicine Counties

Fees
Services will be billed to insurance or a sliding fee scale is available if a client is eligible. Accepts private insurance, Medical Assistance, and other third party forms of payment.

Offers adult mental health support services to those living with a serious mental illness, such as connection to resources and organized activities MORE INFORMATION A coordinated care and treatment program that provides a wide range of individualized treatment, rehabilitation and support services to persons with serious and persistent mental illness. Assists individuals to live as successfully and independently as possible in their own communities by supporting independent living and coping skills.
