Eligibility
Anyone receiving state provided or state licensed services or treatment for developmental disabilities, mental illness, chemical dependency or an emotional disturbance.

Regional Ombudsman staff are located across the state, have experience working in specific geographic regions, and are most familiar with the resources available in their areas. An ombudsman is a neutral party who investigates consumer complaints. They work with both the consumer and the organization to resolve consumer issues and improve customer service. Minnesota State law protects people from retaliation that make complaints in good faith. Medical Review - Death and Serious Injury Reporting
  • An agency, facility or program must report to this office the death or serious injury of a client within 24 hours of the incident
