Transportation program that provides rides for people in and around their community. Learn more about transportation costs that are covered through waivers in the Community-Based Service Manual. MORE INFORMATION Provides long distance non-emergency medical transportation and relocation (state to state) for patients, older adults and people with disabilities. They will transport to and from a variety of facilities, including private residences, assisted living or nursing homes, hospitals, rehabilitation centers and hospice. A family member or loved one may ride along at no extra charge. Offers long distance wheelchair transport, stretcher transport, and ambulatory transport for:

• Patients with conditions such as Alzheimer's, paralysis, stroke, Parkinson's, etc.
• Disabled and handicapped individuals - including those in a wheelchair or a stretcher
• Elderly persons - including those with medical conditions
• Provides a bed-to-bed service
• Hospital transfers
