Services are directed at maintaining or improving a person's ability to self-care. This includes health, social and nutritional services. Services are directed toward the achievement of specific outcomes identified after assessment in a service plan.
This service may be provided either in-person or remotely.

Supervised group program for older adults and adults with disabilities
Program offers:
- Individualized care plans
- Supervised daily living activities
- Group socialization
- Enrichment activities including arts and crafts, music appreciation, dance, creative writing, photography and other memory enhancing activities
- Assistance for non-English speakers
- Exercise and therapeutic physical fitness programs
- Rehabilitative services, including balance and mobility classes
- Social work services
- Respite and support groups for caregivers
- Nursing care
- Vital signs and medication monitoring
- Meals and snacks including breakfast, hot lunch and two afternoon snacks
- Transportation
