Eligibility

Unable to transport non-ambulatory clients Service is available for wheelchair users if client is able to transfer to the car, and wheelchair is foldable and able to stow in trunk They can accommodate walkers, canes, oxygen, child car seats, and similar items

Application process

It is best to make your appointments a couple days ahead of time, if possible. Transportation hours: 6:00am - 6:00pm, Monday - Friday

Fees

Accepts private pay, Medical Assistance, Blue Cross Blue Shield, Health Partners, Prime West, Medica, Humana, and waivered services
