Information, guidance and support for family and other informal caregivers, typically through in-person meetings, over the phone or using online communications. MORE INFORMATION Resources for Enhancing Alzheimer's Caregiver Health (REACH) is an evidence-based caregiver consultation service. This service focuses on those who care for someone with Alzheimer's disease or a related dementia. Helps caregivers manage behavioral concerns, reduce caregiver burden and improve or sustain caregiver physical and emotional health. It is delivered over a two to three month time period including four sessions with the caregiver either face to face or by telephone including:
  • Education about Alzheimer's Disease, dementia, safety and health care issues
  • Problem solving for target behavior and issues and creation of plans to address issues
  • Stress management techniques based on the caregiver's needs
  • Additional sessions may be provided based on caregiver need or desire
