Although this service is provided by a pharmacist, you do NOT have to be a patient to receive this service. The pharmacist can work with your health care providers, no matter where you receive health care. You may be eligible for medication management services at a reduced cost, in your home or at a Hennepin Healthcare clinic, if you:

• Are 60 years or older
• Live in Anoka, Carver, Dakota, Hennepin, Ramsey, Scott or Washington counties
