Must meet all of the following: * Be uninsured or underinsured * Annual income at or below 275% of the federal poverty level * Serves Goodhue Wabasha counties

Application process

Call the Care Clinic between 8:00pm - 4:00pm, Monday - Thursday. Tuesday walk-in clinic: Registration is from 4:00pm – 6:30pm, and the clinic is open until 8:00pm. Services are provided on a first come, first serve basis. Clinic is located in the new Seminary Professional Building at Mayo Clinic Health System. Enter clinic through Door #1 at the end of Hill Street.
