- Must be currently insured and have coverage for the medication seeking financial assistance
- Have a confirmed diagnosis and treatment plan
- Must live and receive treatment in the United States
- Income must fall at or below 400% or 500% of the Federal Poverty Guideline

Complete an online registration or by calling (866) 512-3861 and asking to speak with a Co-Pay Relief Specialist.
Program Specialists are available to personally guide you through the application and enrollment process.
