DESCRIPTION: Comprehensive review of all current medicines by a licensed pharmacist to help get the most benefit for your health and lifestyle. Review includes all prescription medications, herbal supplements, and over-the-counter medicines to help you understand: * If you are taking only the medicines and amounts you need * How your medicines interact with each other * If all your medicines are safe and effective * If there are any lower-cost options Review will also answer questions and provide education on your medicines.

Included for Medicare Part D subscribers meeting certain eligibility criteria ADDITIONAL INFORMATION: Private pay and many types of insurance are accepted. * A contribution is requested from all participants based on income level if not covered by insurance. However, no one will be turned away for inability to pay.

Talk to your pharmacist about eligibility requirements ADDITIONAL INFORMATION: Anyone can use this service. Although this service is provided by a pharmacist, you do NOT have to be a patient to receive this service. The pharmacist can work with your health care providers, no matter where you receive health care. You may be eligible for medication management services at a reduced cost, in your home or at a Hennepin Healthcare clinic, if you: * Are 60 years or older * Live in Anoka, Carver, Dakota, Hennepin, Ramsey, Scott or Washington counties
