Case managers help people to plan, access, evaluate and coordinate services from their service providers. Plans are customized to each person's own needs and preferences. MORE INFORMATION Provides assistance to Veterans and their families to reintegrate after discharge from active military service The Post 9/11 Transition and Care Management program provides the coordination of care to help Veteran access health care needed and to assist in the transition back to life as a civilian.
