Minnesota residents - guidelines for aid qualifications will be sent with the application.
Application process
  • Referring Professionals are directed online to request an application
  • A Referring Professional may be a social worker, nurse, school counselor, teacher, principal, social worker, clergy, health care technician, etc.
  • Applications cannot be submitted by the applicant, the vendor, or by a relative or friend
