Program that works on behalf of individuals and/or groups to make sure that they receive the benefits and services for which they are eligible MORE INFORMATION Provides one-on-support for adults with cognitive and developmental disabilities to meet their goals of independence Services include:
  • Housing assistance
  • Finding food support programs
  • Help applying for public programs
  • Shopping, budgeting, financial and application assistance
  • Home visiting and home management skills
  • Safety planning
  • Access to medical support and follow-up on medical directives
  • Employment and volunteer readiness and placement
  • Support with legal issues, daycare, school and landlord/tenant issues
  • Ongoing one-on-one daily living skills counseling and general support
