Eligibility

The child or adult in need of services must complete an application and have an assessment completed by a mental health professional within the past 180 days.

Case managers help people to plan, access, evaluate and coordinate services from their service providers. Plans are customized to each person's own needs and preferences. Learn more about case management costs that are covered through waivers in the Community-Based Service Manual. MORE INFORMATION Mental health targeted case management is a voluntary service provided by a case manager or social worker for people who meet certain eligibility requirements. Provides or help get access to: * Assistance in gaining support from family, friends, neighbors, churches, etc. * Assistance to obtain financial resources such as the Medical Assistance option of Tax Equity and Fiscal Responsibility Act (TEFRA) * Coordination with school programs * Crisis resources * In-Home Family Skills and Therapy * Mentoring * Respite care * Summer camp * Support groups
