ADDITIONAL INFORMATION:
If you are less than 65 years old or disabled and have MNCare, Medicaid, Medical assistance, or equivalent private insurance plans and have multiple chronic conditions, take multiple prescriptions, and are not living in skilled nurse facilities.
If you are a state employee and have State Employee Group Insurance Program with diabetes you are able to have a free medication review.
If you are over 65 and have Medicare Part D, find if you are eligible for free Medication Therapy Management (MTM) service by referring to your plan.

Comprehensive review of all current medicines by a licensed pharmacist to help get the most benefit for your health and lifestyle.
Review includes all prescription medications, herbal supplements, and over-the-counter medicines to help you understand:
- If you are taking only the medicines and amounts you need
- How your medicines interact with each other
- If all your medicines are safe and effective
- If there are any lower-cost options
- Medication related action plan and comprehensive review of master list of medications (herbal, prescription and Over the Counter) and side effects to watch for
- Long-term follow-up, quarterly or more frequently
- Education for drug-related issues for patients and their caregivers
- Coordinate with patient's primary care provider regarding patient's medication regimen
- Screening for early signs and symptoms of hypertension, diabetes, and dementia
- Help reduce costs by tracking dietary supplements for duplicates and dosage
- Prescription medication misuse (addiction, physical dependence, adherence) consultation
- Personalized dosing of your medication
- Helps you to switch to lower cost medications or discontinue prescription medication
