Must be a UCare plan member including: * Prepaid Medical Assistance Program (PMAP) * Minnesota Senior Care Plus (MSC+) * UCare Connect Special Needs BasicCare (SNBC) * UCare Connect + Medicare * UCare's Minnesota Senior Health Options (MSHO) * Children on MinnesotaCare * Adults on MinnesotaCare (limited to colonoscopy and mammogram services)

* Bus passes may be given up to 14 days in advance * Those who do not live on a bus line or cannot safely use public transportation need to call 2 days before an appointment to schedule a ride
