Eligibility

Medicaid enrollees diagnosed with a Serious Mental Illness (SMI) or Emotional Disturbance (ED). ADDITIONAL INFORMATION:
  • Must have a current diagnosis of mental illness or substance use disorder
  • Must have Medical Assistance

Site Service Area

Most programs serve Itasca County. Service area may be dependent on program. CCBHC services available in Itasca and Koochiching counties.

This agency has been certified to offer Behavioral Health Home (BHH) services. The BHH services model covers all areas of health and wellness for children and adults. This team-based and person-centered approach supports the integration of behavioral health and primary care. Patients receive applicable core services such as:
  • Care management
  • Care coordination
  • Health promotion and wellness
  • Transitional care
  • Patient and family support
  • Referral to community and social support services
