Program that works on behalf of individuals and/or groups to make sure that they receive the benefits and services for which they are eligible MORE INFORMATION Provides information and referrals to Hmong Seniors for:

• Housing
• Medical and financial services, including senior housing, Medicare/Medicaid, social security benefits, food stamps, and energy assistance

Additional services include:

• Person-centered case management services
• Transportation for shopping, medical appointments, or other activities
• Social and educational program activities
• Volunteer companions

Bilingual staff are also able to provide interpretation and translation services for Hmong Seniors.
