Case Management for Homeless Families

Eligibility

Homeless families (one family member must have a disability) residing in a Hennepin County homeless shelter Preference given to residents of People Serving People and St. Ann's.

Application process

Priority is given to residents currently residing in Hennepin County funded shelters referred by the Hennepin County Shelter Team. Individuals residing in non-county shelters will be placed on a waiting list and if Hennepin County cannot make a referral, this wait list will be consulted.

Description

Intensive case management services assist homeless families to locate and stabilize their housing for those residing in a Hennepin County homeless shelter. Services: * Intensive case management services which includes referral to housing and other community and county resources needed for family to maintain stable housing * Advocacy, budgeting, home management, transportation, linkage to mental health services, etc. Families in a Hennepin County shelter will be referred to the shelter team for screening for Project Connect.
