Case managers help people to plan, access, evaluate and coordinate services from their service providers. Plans are customized to each person's own needs and preferences. Learn more about case management costs that are covered through waivers in the Community-Based Service Manual. MORE INFORMATION Provides:
  • A comprehensive assessment of a person's strengths and needs in the areas of: physical, cognitive, emotional, social, financial, and legal situations
  • Develops an individualized Care Plan
  • Navigates senior housing options, accompany on tours of your choice, and coordinate and facilitate the entire moving process
  • Provides medication management
  • Provides client/family counseling and support
  • Facilitates and coordinates financial or legal services, including estate planning, guardianship, conservatorship and power of attorney
  • Provides medical oversight and care coordination with family and doctors
  • Holistic Nursing Care services along with traditional medical and social services
  • Screens, arranges and monitors in-home care or related services
  • Communicate with long distance family members
  • Offers a professional travel companion through the Safe Passage Program
  • 24/7 mental health crisis intervention
  • Assists with psychiatric diagnosis differentiation
  • Accesses appropriate and cost-effective psychiatric care
  • Partner with families, doctors and other mental health professionals to coordinate care
  • Problem solves matters involving probate and mental health court
