Check online to ensure a participating provider is in the area by adding your zip code. An application is available online. The staff will contact the patient if the application is accepted. Patient's doctor must fax verification of current cancer treatment. The foundation will contact the provider to set up the service. The maid service will contact the patient to schedule the cleanings.
