Talk to your pharmacist about eligibility requirements ADDITIONAL INFORMATION: Anyone can use this service. Although this service is provided by a pharmacist, you do NOT have to be a patient to receive this service. The pharmacist can work with your health care providers, no matter where you receive health care. You may be eligible for medication management services at a reduced cost, in your home or at a Hennepin Healthcare clinic, if you: * Are 60 years or older * Live in Anoka, Carver, Dakota, Hennepin, Ramsey, Scott or Washington counties

Included for Medicare Part D subscribers meeting certain eligibility criteria ADDITIONAL INFORMATION: Private pay and many types of insurance are accepted. * A contribution is requested from all participants based on income level if not covered by insurance. However, no one will be turned away for inability to pay.
