Case managers help people to plan, access, evaluate and coordinate services from their service providers. Plans are customized to each person's own needs and preferences.   • Referrals to clinics, mental health providers, legal services, English classes
  • Assistance with basic needs, such as housing, furniture, household items, food, clothing, employment
  • HIV counseling and referral

  • Culturally and linguistically appropriate HIV-educational materials
