Provides assistance connecting older adults to services offered by programs in the Living at Home Network. Supports local programs that help elders to stay in their own homes by organizing volunteer support, coordinating existing services, and offering exercise classes , health promotion and other activities. Programs coordinate with existing services to reach seniors and add missing professional and volunteer services to enhance the life of seniors in the community. By receiving in-home social support and health care, elders are enabled to stay in their homes for extended periods of time, preserving their quality of life and retaining their community networks, while at the same time avoiding expensive nursing home costs. The Living at Home Network website lists the Minnesota sites for programs in local communities, to help seniors and caregivers find the closest program. The website also provides a map showing the Twin Cities metro service area boundaries.. Services typically offered by area programs: * Provide information on topics such as housing, food resources, and financial assistance * Help identify risk factors living alone * Offer visits from volunteers * Social gatherings and activities * Exercise programs and health education, home safety checks, blood pressure checks, access to affordable in-home nursing care * Transportation - for health-related appointments * Chore assistance
