DESCRIPTION: Access, consultation, assessment and service planning for seniors and persons with developmental disabilities of all ages (from birth on) Services include: * Getting help to stay home * Assessment of needs * Information and consultation about community resources and county, state and federal programs for seniors and persons of all ages with developmental disabilities * Short-term service planning and case management to help individuals and families develop a plan of care * Assistance with applying for public benefits * Access to home care, and case management * Access to information on specialized housing and funding options for children and adults * Help for adults to maintain their mental health and social well-being
