Eligibility

* Live in the community and not a facility such as a nursing home * Be someone that can direct their own care or have a representative who can direct care on their behalf * Be enrolled in one of Minnesota's health care programs including: Medical Assistance (MA), Home and Community-Based Services (HCBS) waivers or the Alternative Care (AC) program and some people on MinnesotaCare * Have an assessment that determines eligibility * Have selected the CFSS agency model

Application process

CFSS services must be found necessary as determined by an assessment of needs. Contact your local county agency or tribal nation if you are on a waiver, Alternative Care or on state Medical Assistance and not enrolled in a health plan for seniors. If you are a senior enrolled in a health plan, you must contact your health plan member services.

A support worker is someone trained to help people that need support to take care of their day-to-day activities. Support workers help people live and work within their own communities. These support workers help with daily living needs because of disease, a disabling condition, or complications related to aging. Activities they can help with include: * Daily living activities * Health related activities * Behavior observation and monitoring Community First Services and Supports (CFSS) provider agencies support people eligible for CFSS who select the CFSS agency model by employing their workers and performing all employment tasks such as: recruiting, hiring, training, supervising and evaluating support workers (they do not support individuals in the CFSS budget model). The individual receiving care is still responsible for directing their own care and has a say in selecting their support worker, setting worker's schedules and can participate in the training and supervising of their support worker.
