Aging life care managers act as guides and advocates for older people and their families. Also known as senior care managers or geriatric care managers, these experts help older adults live at home rather than in a facility. They can provide any of the following services, although this is not an exhaustive list:
  • Assessment and monitoring
  • Planning and problem-solving
  • Match needs and services
  • Assist families with care coordination
  • Crisis intervention and updating levels of care as needs change
These professionals typically have a background in social work, psychology, nursing, or gerontology. They also have specific expertise in assessing, planning, and serving the needs of older adults. MORE INFORMATION Care managers meet with individuals in their home or at one of the community centers and provide support and assistance. They complete assessments, meet with the senior, gather input from caregivers and create goals together. Based on the goals, care managers inform the senior and caregivers about the services that are available in the community and help them access them. Keystone care managers can accompany seniors to doctor's visits, advocate for the senior's health needs and assist the senior with health-related communications between primary care providers, specialists, home health care, pharmacy, hospital staff and out-of-town family members. Care managers can also assess levels of caregiver stress and provide access to supportive services.
