Services are directed at maintaining or improving a person's ability to self-care. This includes health, social and nutritional services. Services are directed toward the achievement of specific outcomes identified after assessment in a service plan. This service may be provided either in-person or remotely. MORE INFORMATION This is a community based group program designed to meet the specific needs of Cambodian elders with functional impairments through an individual care plan. Services include culturally sensitive recreational, therapeutic, and social activities among peers, such as: * Translation * Group Discussion * Storytelling * Arts, Crafts, and Music * Supervised, Structured Exercise * Case Management * Transportation Arrangements * Chronic Disease Management * Assistance with Activities of Daily Living (ADL) * Health Education: Diabetes Classes, Nutritional Classes * Nutritionally balanced and culturally appropriate morning and noon meal and afternoon snacks
