Click the “Renew License” button on the right, then click “Register to Access Site” to create your new login account.
Then make a secure payment through US Bank with a MasterCard, Visa or Discover credit card. This account will be used for future online activity, so maintain a copy of your login information.
Print a paper Renewal Application , and mail it in with a check with the late fee.
In order for the Board to change the name in which your LD or LN was issued, you are REQUIRED to submit one of the following:
1) a marriage certificate specifying the name change following marriage;
2) a divorce or dissolution of marriage decree specifying the name change, or;
3) other court order specifying the name change.
Merely copy the document and fax, scan and email, or mail the copy to the Board office. There is no fee for this service. Your name will be updated in the database. During renewals, the board staff will update your renewal certificate with the name change. It is best to send in the name change documents first and then renew online once the name change has occurred in the database. The name change occurs on the website the following day for the public licensee search.
To avoid late fees, your renewal must be postmarked on or before November 30, 2014 or online transactions must be fully completed by midnight, November 30, 2014.
To help you prepare for the end of the continuing education cycle please review the table on the Continuing Education page, under the Licensees tab above, that shows the amount of CEU's required by license number that will have to be submitted by November 30, 2015.
