Organization Monthly Reports for
Schedules A, C, and F

Starting with the July 2012 monthly reports [due August 20, 2012], organizations are:
  • required to electronically submit their monthly Schedules A, C, and F to the
    Gambling Control Board [Board], and

  • no longer required to submit the [paper] Schedule A, C, and F reports to the Minnesota Department of Revenue.

The first electronic report is due August 20, 2012.

The electronic format requires the use of Microsoft Excel.  If you do not have Microsoft Excel software, read the instructions for other options.

If you are currently using software provided by a lawful gambling software vendor, contact your vendor for information on submitting the reports.

Reminder:  If you are reporting or amending Schedules A, C, and F for months prior to July 2012, you must use the current paper forms to submit the report to the Department of Revenue.

Electronic format - Depending on your computer's operating system, use one of the electronic formats below, starting with the July 2012 reports due August 2012.

The electronic format reflects the new tax rates and forms that go into effect on
July 1, 2012.  Until then, you must use the current paper forms for Schedules A, C, and F.

If your organization has five or fewer sites:

  • Your computer operating system is Windows 7 or later;
  • You use Internet Explorer 9 or later; and
  • You use Excel 2010 or later.

Email Electronic File to Gambling Control Board

  1. Open your email program (for example, Yahoo Mail, Gmail, Windows Live, etc.)
  2. Attach the electronic file to your e-mail.  This process differs depending upon which email program you use
  3. In the subject line type license #, month and year of report example: 12345-07-2012
  4. Send email to gcbreports@gcb.state.mn.us
  5. You will receive an automated reply confirming that we received your reports.
