Notification of Lead Hazard Reduction
Applications are available on the MN Dept. of Health (MDH), Lead Professionals section, website at Lead Poisoning Prevention Forms.
Requirements:
1. A Minnesota licensed lead supervisor must notify the commissioner at least 5 calendar days before conducting lead hazard reduction, whether ordered or voluntary.
2. If the work was ordered by an assessing agency other than MDH, notice must be provided to the assessing agency as well.
3. Any changes to the original notification must be submitted as an amendment no later than the day the changes occur.
Fee:
No fee.
Period of Issuance:
Notification expires on the end date stated on the notice or any amendment of the end dates.
Length of Determination Process:
The notification form must be submitted at least five calendar days before the beginning of the project.
