Application forms are available on the MN Dept. of Transportation (Mn/DOT) website, Utility Agreements and Utility Permits section at Permit Forms.

Requirements:

A surety bond usually required and retained for one or two years after installation. Wording for bonding (individual or continuing) is available on the Mn/DOT website at Utility Permit (Individual Bond) or Utility Permit (Continuing Bond).
