HIV case management is a program that offers help with things important to staying healthy. Case managers can help you overcome barriers to accessing services, set and achieve goals and find resources. Their job is to work with you to help you live successfully with HIV.



Living with HIV can be challenging. HIV case managers are able to help you connect to care, support services and financial assistance. Their job is to work with you to support you to live successfully with HIV.

Case management is available through several HIV/AIDS organizations as well as through many hospital settings. Select a clinic and call to find out more information.
